The name comes from the term "sguazzo," "guazzatoria," or "dai guadi," which were mandatory to cross when traveling on foot or by boat from Tortonese to Lomellina, Oltrepò Pavese, or Pavese, separated by the Tanaro and Scrivia rivers here converging into the Po.

The settlement was founded in the 13th century and administratively depended on the municipality and principality of Pavia, despite its proximity to Tortona. Until 1100, it was ruled by the noble lordship of Corti from Voghera, to whom Matteo Visconti assigned, in 1314, the castle of Montemerso, control of the crossings over the Po, and protection of the ports. At the end of the 15th century, Michele dei Nobili of Guazzatoria established a new transit to the island of Garda. In 1507 the Corti nobles still lived in the area, while a century later, in 1644, they were transferred to Pavia. In 1700, it was feudalized to the Biglia nobles.

Historically and culturally linked to Pavese since its beginnings, in 1748 Guazzora passed to Piedmont and was integrated into the province of Voghera from which it was administratively separated with the royal edict of November 10, 1818, when it was united with the Tortona District in the Sale mandament.
