The name Maltignano derives from the Roman Publio Maltino Basso, who owned the land of the current municipal territory. The first time the name of the municipality appeared on the map was thanks to Charlemagne, who, when he came down to Italy, donated some castles south of the Tronto to the diocese of Ascoli Piceno, including Maltignano and the nearby centers of Ancarano and Sant'Egidio alla Vibrata, an ancient fortress.
